Why does acne hurt?

Acne starts when oil and dead skin cells block a hair follicle, the opening where a hair grows. Pressure inside the blockage can break the follicle wall and release its contents into surrounding skin. Your immune system responds with white blood cells, producing swelling, pus and tenderness. Acne starts inside blocked follicles, so treating it as a surface cleaning project gives the soap the wrong job.13

Does acne hurt?

Some acne hurts; some does not. The type of spot matters:

  • Blackheads and whiteheads sit closer to the surface and usually are not swollen or painful.
  • Papules are small raised bumps that can feel tender or sore.
  • Pustules are raised bumps containing pus and can be painful.
  • Nodules are large, hard lumps beneath the skin. Cysts are deeper, fluid filled lumps. Both can hurt, and cysts carry a particularly high risk of permanent scarring.

Why is my back acne so bad?

The middle of the back has many large oil glands, making it an acne prone area. Hormones, genetics, certain medicines and clothing that traps heat can contribute to acne severity. Sweat and rubbing from a backpack can also aggravate back breakouts. These factors can worsen acne without explaining every painful lump; a clinician can assess the type and severity.45

Back acne covers an area that is hard to reach, which is an inconvenient place to put a daily maintenance job. A back lotion applicator or help from someone else can make applying treatment easier.5

What should you do about painful back acne?

While arranging care for deep, painful spots, reduce avoidable irritation:

  1. 1Wash gently with a mild cleanser and lukewarm water. Do not wash the affected area more than twice daily or scrub it.
  2. 2Shower and change clothes as soon as possible after sweating. Choose loose, breathable clothes and wash workout clothes after each use.
  3. 3Remove friction where possible. Switch from a backpack to a handheld bag, and stop using loofahs, back brushes and abrasive scrubs.
  4. 4Leave spots alone. Picking or squeezing can increase pain, worsen inflammation and cause scars.

For mild back acne without deep, painful bumps, a daily benzoyl peroxide foam wash can help control breakouts. Leave the wash on the back for two to five minutes before rinsing. Benzoyl peroxide can cause dryness, irritation or peeling and bleach fabrics. Ask a pharmacist about choosing a treatment; do not rely on a wash alone for deep nodules.2

When should you see a doctor?

Book an appointment for deep or painful lumps, widespread acne, scarring, or breakouts that are worsening despite treatment. Depending on severity, a doctor can offer prescription creams or oral medicines, including antibiotics or isotretinoin. Skin that becomes hot, swollen and red also needs assessment for infection; redness can be harder to see on brown and black skin.56

Large bumps that connect beneath the skin or drain pus need medical assessment. These can occur in acne conglobata, a rare, severe form that can cause deep scarring. Pain alone does not identify this condition. Seek urgent medical care if severe acne appears abruptly with ulcerated spots, fever, painful joints or feeling generally unwell.47

How long does treatment take?

For back acne suitable for home treatment, improvement can start in six to eight weeks, while complete clearing can take three to four months. If there is no difference after six to eight weeks, see a dermatologist. That waiting period is for mild breakouts, not a reason to delay care for deep, painful lumps.2
