What is causing the burning?

Your skin barrier is the outer layer that keeps water in and irritants out. Harsh soaps, scrubbing and too much exfoliation can damage it. Stinging when you apply skincare, especially alongside dryness, flaking or tenderness, can be a sign of barrier damage. Retinoids and benzoyl peroxide can also cause irritation, so the hand cream is not necessarily the only product involved.23

Fragrance is the most common cause of contact dermatitis from cosmetics, and preservatives are another common cause of cosmetic allergy. Contact dermatitis means skin inflammation triggered by something touching the skin. An irritant directly damages the skin; an allergen triggers an immune reaction. Burning alone cannot tell you which is happening. An allergy can develop even after years of using the same cream comfortably.45

Does immediate burning mean an allergy?

Not necessarily. Strong irritants can cause symptoms within minutes or hours, while weaker irritants can cause problems after repeated use over days or weeks. Allergic contact dermatitis usually appears more than 12 hours after exposure and peaks around 48 hours. Hives, another type of contact reaction, can appear within minutes to an hour. The timing helps a clinician investigate, but does not identify the cause by itself.4

What should you do now?

  1. 1Stop applying the hand cream to your face. Avoiding the substance causing the reaction is the main way to prevent further contact dermatitis.
  2. 2Rinse the affected skin as soon as possible with warm water and an emollient cleanser, a moisturizing soap substitute. Avoid hot water.
  3. 3Use gentle skincare while the irritation settles. Choose a mild cleanser without soap or fragrance, and avoid scrubbing or excessive cleansing.
  4. 4Replace the cream with a facial moisturizer without fragrance or botanical extracts. Apply moisturizer after washing, while your skin is still slightly damp.

What moisturizer should you use instead?

A gel or lotion is generally a better facial choice unless your skin is very dry, so the thickest texture does not automatically win the moisturizer audition. Look for moisturizing ingredients such as ceramides and fatty acids, which help support the skin barrier. Apply moisturizer consistently after washing rather than repeatedly trying the cream that burns.13

"Unscented" can still mean a product contains fragrance used to mask other smells. "Fragrance free" is the better starting point, but other ingredients can still cause allergies.14

If you already have rosacea, burning, stinging or itching from skincare can signal irritation that triggers a flare. Avoid products containing menthol, camphor or sodium lauryl sulfate, and ask a dermatologist for a suitable skincare plan. Burning after hand cream alone is not enough to diagnose rosacea.6

When should you get medical help?

Burning can happen before a visible rash, so do not wait for redness before stopping the cream. Contact dermatitis can also cause itching, swelling, cracking or blisters; on darker skin, discoloration can look brown, purple or grey rather than red. See a doctor for persistent, recurring or severe symptoms. A dermatologist can use patch testing to investigate an ingredient allergy. Avoiding the trigger usually helps dermatitis improve, but there is no single recovery time for every reaction.457

Get emergency help for difficulty breathing, throat swelling or collapse after applying a product. These can be signs of a rare but serious allergic reaction.4
