Why a moisturizer can suddenly feel uncomfortable

Adapalene commonly causes dryness, redness, scaling and burning or stinging early in treatment. Dry skin is damaged skin, and skincare products can irritate it. A moisturizer that previously felt comfortable can therefore become uncomfortable during treatment. This is a general explanation, not a diagnosis of an allergy or a particular skin condition.234

Moisturizer still has a useful role: daily use can help skin tolerate drying acne medicines such as adapalene. Its job is to make treatment easier to tolerate. But if a particular moisturizer causes a reaction, stop using that product and ask a pharmacist for an alternative.25

Which moisturizer should you try?

Choose a gentle, fragrance free moisturizer, which does not need to double as a perfume sample. For acne prone skin, look for "noncomedogenic," "won't clog pores" or "oil free" on the container; these descriptions mean the moisturizer is unlikely to cause acne. "Unscented" is not the same as "fragrance free," because a product can hide an odor without retiring the ingredients responsible for it, which is quite a loophole for a word about smelling nothing.45

Avoid salicylic acid, alpha hydroxy acids and glycolic acid while using Differin because they can worsen dryness and irritation. Skip abrasive cleansers, scrubs and products with strong drying effects. Do not add another retinoid. If another acne medicine is prescribed, ask your pharmacist how to combine it with adapalene rather than changing the prescription yourself.36

How to apply Differin and moisturizer

These steps follow the US Differin Gel 0.1% directions; UK prescription guidance also allows a gradual start for sensitive skin, so follow your own label and prescriber's instructions.67

  1. 1Wash gently with a mild cleanser and pat dry. Do not scrub.
  2. 2Apply a thin layer of Differin to the whole acne affected area, not just individual pimples. Avoid eyes, lips and mouth, and do not apply to broken, sunburned or eczema affected skin.
  3. 3Let the adapalene absorb, then apply moisturizer. Differin's usual schedule is once daily, in the morning or evening; applying extra does not improve results.
  4. 4Use sunscreen when going outdoors and limit sun exposure. If applying Differin during the day, let the gel dry before applying sunscreen.

Tell your doctor if your skin stings, burns or becomes irritated; they can advise whether to use adapalene less often. UK guidance describes starting two to three times weekly and increasing as tolerated. Outside the Differin application, moisturizer can be reapplied whenever skin feels dry, including after washing while skin is still damp.1456

How long should irritation last?

Early irritation usually eases after about four weeks of continued use. That adjustment period is different from acne improvement, which can take up to three months. Brief warmth or stinging directly after applying adapalene can settle quickly, but persistent moisturizer burning deserves medical advice rather than being treated as proof the acne medicine is working.147

When should you stop and get help?

Stop Differin and ask a doctor before restarting if irritation becomes severe or does not improve. Get urgent medical help for hives, swelling of the face, lips, tongue or throat, or difficulty breathing. If everything you apply burns or stings, tell a dermatologist. Ask a doctor before use during pregnancy or breastfeeding, and stop Differin if you become pregnant or plan to become pregnant.1467
