What is happening in your skin?
Tretinoin changes how cells behave inside hair follicles, the openings where acne plugs form. It makes those cells less likely to stick together and increases their turnover, helping prevent plugs and clear existing ones. Irritation is a separate effect to manage, not a reason to apply more.3
The skin barrier is the outer layer that helps retain moisture and keep irritants out, without needing to be called a moisture fortress. When that barrier does not work well, skin holds less water and can become rough, scaly or cracked. Moisturizers help improve barrier function and reduce water escaping through the skin, a less theatrical goal than turning back time.2
How to care for skin while using tretinoin
Use gentle dry skin care alongside your prescribed treatment:
- 1Wash with a gentle, fragrance free cleanser and lukewarm water. Avoid hot water, scrubbing and rubbing with a towel.
- 2Pat gently and apply moisturizer promptly after washing, while skin is still damp. Reapply during the day whenever skin feels dry.
- 3Use tretinoin at bedtime as directed by your prescriber. Keep it away from your eyes, mouth and the corners of your nose. If irritation develops, ask whether to use it less often or pause treatment.
- 4During the day, apply a broad spectrum, water resistant sunscreen with SPF 30 or higher. Seek shade and use protective clothing.
For pronounced dryness, choose a cream or ointment rather than a lotion. Creams and ointments hold more moisture than lotions, so the texture matters more than the jar's ambitions. Look for fragrance free products; an unscented product can still contain chemicals used to hide odors that irritate dry skin. Ceramides help strengthen the protective barrier, and hyaluronic acid is another moisturizing ingredient to look for.456
What makes the dryness worse?
Do not scrub off flakes or add exfoliation to remove them. Exfoliating already dry skin can remove more of its surface and create further damage. Medicated or abrasive cleansers, strongly drying cosmetics, astringents and products high in alcohol can also add irritation. Check with your prescriber before combining tretinoin with other topical medicines, especially products containing salicylic acid, sulfur or resorcinol. Cold weather and wind can aggravate treated skin, too.36
How long does the dryness last?
There is no fixed adjustment period for tretinoin dryness. As general dry skin guidance, effective home care often brings improvement within two weeks. If dryness continues or worsens, have it assessed rather than waiting indefinitely. Treatment benefits follow a different timetable: topical retinoids can take 12 weeks or longer to improve acne, and tretinoin's effects on sun damage and fine wrinkles require longer use.157
When should you stop or contact your prescriber?
Stop and contact your prescriber for a reaction suggesting sensitivity or chemical irritation. Severe redness or peeling can require less frequent treatment or a temporary pause. Tell your dermatologist if moisturizer burns or stings, or itching interferes with daily life. Do not apply tretinoin to sunburned skin until it has recovered.346
Eczema needs particular caution because tretinoin can cause severe irritation on affected skin. Cream formulations are generally less irritating than gels, so discuss the formulation if dryness is making treatment difficult. If you are pregnant or planning pregnancy, discuss tretinoin with your prescriber before using it; topical retinoids carry pregnancy precautions.13
