Is it the witch hazel or the rest of the formula?

Both deserve attention. Witch hazel is an astringent, meaning it shrinks or tightens tissues. Witch hazel pads can cause mild irritation, redness or dryness. Alcohol and fragrance can also irritate skin, so check the whole ingredient list rather than just the name on the front.14

Some witch hazel liquids contain alcohol. For example, Exchange Select witch hazel contains 14% alcohol by volume. An alcohol free formula removes that particular concern, but witch hazel itself can still irritate. Exchange Select lists cleansing and toning among its claims, so one bottle has given itself several job titles.45

Does stinging mean an allergy?

Stinging alone does not tell you whether the problem is irritation or allergy. Contact dermatitis is skin inflammation caused by contact with an irritant or allergen. An irritant directly damages the outer skin layer; an allergen triggers an immune reaction. Fragrance in a personal care product can cause allergic contact dermatitis.36

Watch for itching, tenderness, swelling, unusually dry or cracked skin, hives or blisters as well as burning. Burning can start before a rash appears. Contact dermatitis often develops within hours or days, so a reaction need not happen during application. A rash can look red on lighter skin or dark brown, purple or grey on darker skin.36

What should you do when it stings?

  1. 1Stop using the witch hazel. Avoiding the substance causing contact dermatitis helps the skin clear.
  2. 2Gently rinse the affected skin with warm water and an emollient wash, a moisturizing cleanser, as soon as possible.
  3. 3Apply a fragrance free moisturizer to dry, irritated skin. A pharmacist can help you choose an emollient if symptoms are troubling you.
  4. 4For freshly shaved skin, use a cool washcloth for comfort and pause shaving, waxing and plucking until the irritation clears.

After shaving, choose soothing care rather than another astringent. Aloe vera gel can cool uncomfortable razor burn, although it does not cure it. Avoid fragranced products and alcohol on the irritated area. Razor burn can settle within hours or take a few days; worsening or persistent symptoms need medical attention.1

Does the type of witch hazel matter?

Witch hazel comes as a liquid and as pads or wipes, so the name alone is doing remarkably little paperwork. Follow your own package directions. Exchange Select liquid is for external use, with instructions to apply as needed and avoid the eyes. If it gets into your eyes, rinse thoroughly with water.45

Medicated witch hazel pads are used for minor rectal pain and irritation, but ongoing irritated skin around the anus calls for a different approach: avoid witch hazel and wipes, rinse gently and pat dry. Before using medicated pads, tell your care team about pregnancy, breastfeeding or previous allergic reactions. Ask before adding other skin products to the same area.24

When should you get medical help?

See a doctor for severe, recurring or persistent symptoms. Exchange Select directs users to stop and ask a doctor if the condition worsens or symptoms last more than seven days. Seek urgent medical help for swelling of the face, lips, tongue or throat.345

For a rash that keeps returning, tell a dermatologist which skin products you use and when symptoms appear. The dermatologist can examine the rash and arrange patch testing when needed to identify an allergy. Avoid repeatedly trying the same product while waiting to find the cause.7
