What else can cause chapped lips?
Cheilitis means inflammation of the lips. Possible causes include eczema, reactions to medicines such as retinoids, thyroid disorders, some autoimmune disorders, and deficiencies of iron or B vitamins. Cracking at the corners of the mouth can involve a fungal infection. Persistent symptoms deserve assessment rather than guessing which condition or supplement fits.1
An itchy, painful rash on or around the lips, with dryness, flaking or splitting, can be contact dermatitis: inflammation caused by irritation or an allergic reaction to something touching the skin. Lipstick, gloss, balm and wind instrument mouthpieces are possible triggers. Toothpaste ingredients can also cause allergic lip inflammation.23
What if a lip product seems to be causing it?
If you suspect a contact rash from lip products, stop applying them until the rash clears. Then add back one product a week. A rash returning within seven days of restarting a product makes that product a likely trigger. If you cannot identify the cause, a dermatologist can help. Choose products labeled "fragrance free" rather than "unscented": unscented products can still contain fragrance that has been masked.2
How to care for ordinary dry, chapped lips
Lip balm helps keep existing moisture from escaping, so its main job is less makeover and more lid.4
- 1Choose a fragrance free, nonirritating balm. For very dry, cracked lips, try a thick ointment such as white petroleum jelly, which seals in water longer than waxes or oils.
- 2Apply balm several times a day and before bed. Reapply after eating or drinking. Stop any product that causes burning, stinging or tingling.
- 3Before going outdoors, apply lip balm with SPF 30 or higher, preferably containing zinc oxide or titanium oxide. Reapply every two hours outdoors, including in winter.
- 4Stop licking, biting or picking your lips. Skip lip scrubs: they remove the protective outer layer and can worsen dryness. Let peeling skin come away on its own.
- 5Drink water to avoid dehydration, use a humidifier in dry indoor air, and cover your lips with a scarf in cold weather. Wash your hands before applying balm, and do not share it.
Avoid menthol, camphor, eucalyptus, phenol, salicylic acid and added fragrance or flavoring while your lips heal. Keeping balm by your bed and in your bag makes regular use easier, without requiring a dedicated lip care department.345
When should you get chapped lips checked?
Expect noticeable improvement within two to three weeks of consistent care. See a clinician if your lips do not improve in that time, get worse or frequently bleed despite treatment. Depending on the symptoms, assessment can include allergy testing, blood tests or a skin biopsy.15
Hot, painful, swollen lips need medical assessment because they can signal infection. A small, fluid filled blister, often preceded by tingling, burning or itching, can be a cold sore. Persistent dry, scaly lips also need checking: actinic cheilitis is a precancerous lip condition associated with frequent sun exposure.156
Soreness inside the mouth, ulcers, blisters or swelling are not simply external chapping. They can occur with stomatitis, meaning inflammation of the mouth lining. Its causes include infection, allergy, injury and medication effects. Tell the clinician about these symptoms and any medicines you take; treatment depends on the cause.7
