Are the bumps actually closed comedones?

Closed comedones are whiteheads: small follicles blocked with oil and dead skin cells, with skin covering the opening. They can look white, yellowish or the same colour as your skin. Comedonal acne commonly affects the forehead and chin, but bumps can also appear on the cheeks, nose and back. Unlike pus filled pimples, these bumps are usually not swollen or tender. Whiteheads and inflamed pimples can occur together, so a routine sometimes needs to address both.3

Which ingredient should you choose?

Topical retinoid, such as adapaleneClears whiteheads and blackheads and helps prevent clogged pores. You want treatment for existing clogs and ongoing prevention.

Choose the active treatment rather than the serum format, because a dropper is packaging, not a treatment plan. Acne medicines also come as creams, gels and lotions. Adapalene is available without a prescription in the United States, while access differs elsewhere; in the UK, adapalene is a prescription treatment. Tretinoin and other prescription retinoids are options to discuss when shop bought treatment is not enough.14

Glycolic acid and benzoyl peroxide are also used for comedonal acne. Benzoyl peroxide targets acne causing bacteria, and a benzoyl peroxide wash can be part of treatment for whiteheads and blackheads. Lower concentrations and wash formulations are less irritating than stronger or leave on options. That makes the choice about more than finding the strongest bottle: the treatment needs to suit the acne you have and be tolerable enough to use.256

How to use treatment for closed comedones

  1. 1Wash gently with a mild cleanser. Cleanse morning and night, remove makeup before bed, and do not scrub.
  2. 2Apply your acne treatment as directed. Treatments that clear comedones are generally spread in a thin layer over the whole affected area, rather than dotted onto individual bumps. Follow the product label for frequency.
  3. 3For adapalene, pat skin dry after cleansing, then spread a thin film over the affected area. It is usually applied once daily at bedtime.
  4. 4Use moisturizer routinely. If a prescribed retinoid irritates your skin, ask your dermatologist whether to apply moisturizer first or adjust the amount or frequency.
  5. 5Protect your skin during the day. Use sunscreen with broad spectrum protection, SPF 30 or higher and water resistance, alongside shade and protective clothing.

What else belongs in the routine?

Choose oil free, water based cleansers, moisturizers and makeup labelled noncomedogenic, meaning formulated not to clog pores. Keep hair products away from your face, and wash your hair routinely. Makeup does not have to leave the routine, which saves acne treatment from becoming a wardrobe policy for your face. Remove it at the end of the day rather than leaving it on overnight.23

Combining acne treatments can address different causes of acne, but a prescription combination is not the same as assembling several products yourself. If you already use prescription acne medication, ask your clinician before adding another treatment. With adapalene, discuss your other skin products with your doctor or pharmacist: harsh cleansers, drying products and products containing alcohol can increase irritation. Do not use hot wax on areas treated with adapalene.17

How long does it take to clear the bumps?

Give whitehead treatment six to eight weeks before judging the response. With persistent comedonal acne, noticeable improvement can take eight to twelve weeks, and clearing can take several months. These are different milestones: fewer new bumps can arrive before existing clogs disappear. Treatment often needs to continue after improvement because comedonal acne can return over many years. If there is no improvement after six to eight weeks, arrange a review rather than repeatedly switching products.356

Adapalene can bring previously unseen pimples to the surface during the first few weeks, and its full benefit can take eight to twelve weeks or longer. Early worsening alone does not mean the treatment has failed. Applying more than directed will not speed results and can irritate skin. Keep following the label, but contact your doctor about burning, stinging or irritation rather than treating every reaction as something to push through.7

Who should be careful with adapalene?

Do not apply adapalene to sunburned, broken or eczema affected skin until it has healed, and keep it away from the eyes, nose and mouth. Tell your doctor if you are pregnant, planning pregnancy or breastfeeding before using it; call your doctor if you become pregnant during treatment. Redness, dryness, scaling and discomfort can occur early in treatment. Seek medical advice for severe or persistent symptoms. Stop using the product and seek urgent medical help for hives, breathing difficulty or swelling of the face, lips, tongue or throat.7

When is a serum not enough?

See a GP or dermatologist if treatment is not helping, you have many spots, or acne is deep, painful, leaving scars or affecting your mental health. Hot, swollen skin with redness can indicate infection and also needs medical assessment; redness can be harder to see on brown and black skin. If you are unsure what the bumps are, get a diagnosis rather than adding more acne products. Folliculitis and perioral dermatitis can resemble acne but need different treatment.14

Persistent whiteheads can need prescription treatment or professional comedone extraction. Do not pick, squeeze or try to extract them yourself: this can damage follicles, cause infection and leave scars. A dermatologist can assess whether stronger topical treatment or a procedure fits the problem. Oral medicines are another option when topical treatment does not work, but they require a medical treatment plan rather than a serum swap.36
