Why combine them?

Benzoyl peroxide reduces acne causing bacteria on the skin. Topical retinoids, including adapalene and tretinoin, help unclog pores and reduce inflammation. Combining different types of topical acne treatment can give better results than relying on one treatment type. Combining treatments is a treatment plan, not an invitation to give every bottle in the bathroom a speaking role.1

Can you apply them at the same time?

Yes, when they are formulated together: Epiduo contains adapalene and benzoyl peroxide. The combination is used for mild to moderate acne, with directions to apply a thin layer once daily in the evening. That gives you a clear example of both ingredients being applied at the same time.24

Separate products need a specific plan because benzoyl peroxide can interact with some other topical acne treatments. Ask your prescriber whether your particular retinoid and benzoyl peroxide product should be applied together or separately. Do not assume the directions for a combined adapalene gel also apply to tretinoin or a cosmetic retinol serum.3

How do you start the combination?

Gentle cleansing belongs in the routine, even if it is the least dramatic bottle on the shelf. Use a mild, nondrying cleanser rather than scrubbing or repeatedly washing your face.5

  1. 1Confirm the application directions for your exact products. Follow the label carefully and apply acne medicines sparingly.
  2. 2Start gradually if irritation is a concern. General acne treatment guidance suggests once or twice a week initially, increasing toward daily use only if tolerated.
  3. 3For a combined adapalene and benzoyl peroxide gel, follow the prescribed schedule and apply thinly in the evening. The usual schedule is once daily, but an introductory schedule can be slower.
  4. 4Watch for redness, dryness or excessive peeling before increasing frequency. If irritation develops, reduce frequency and ask your prescriber how to adjust the routine.

How long does it take to work?

Acne treatment often takes 6 to 8 weeks to show a response, and a treatment review at 12 weeks is a useful checkpoint. Retinoids are better suited to ongoing maintenance than short term spot treatment. If there is no response at the review, discuss changing the treatment rather than simply applying more.356

Who should be careful?

Sensitive skin needs a slower introduction. A lower benzoyl peroxide strength, such as 2.5%, is an option to discuss. Cheeks can become dry and irritated more easily than other facial areas, so every other day application can be preferable there. Benzoyl peroxide can also bleach or discolor towels and clothing.356

Avoid topical retinoids during pregnancy and breastfeeding. Use effective contraception if pregnancy is possible, and discontinue the retinoid if pregnancy occurs. Discuss an alternative acne treatment with your clinician.6

For children, check the exact prescription rather than treating all combination gels as interchangeable. In the US, Epiduo Forte is approved for acne in people aged 12 and older.7

Seek medical advice sooner if acne is worsening, causing painful cysts, leaving scars or causing emotional distress. Acne that is already causing scarring should not wait for several months of unsuccessful self treatment.56
