What can a cream actually change?

Cellulite forms where connective bands pull the skin downward while fat beneath pushes upward, creating dimples. The exact cause is not fully understood. Genetics, sex, age and skin thickness affect its visibility, and lean people can have cellulite too. A cream is an optional cosmetic treatment, not something your health requires.3

Think of a cream as an attempt to improve appearance, not remove the underlying problem. Permanent removal is not on offer, however ambitious the promises on a jar. Caffeine, retinol and other topical treatments have no demonstrated permanent effect on cellulite.1

What before and after results have been measured?

Tested formulaUse and assessmentBefore and after result
ZONE·5 Slimming Zone Smart Silhouette Cream with 3.5% water soluble caffeine and xanthenesTwice daily on thighs and inner upper arms for six weeks; 15 women enrolled and 14 completedAverage visual cellulite score fell by 0.49 points, or 19.8%. Thigh circumference fell by 0.7 cm and upper arm circumference by 0.8 cm.
Experimental nano cream containing 2% caffeine and lanolinActive cream and placebo applied to paired upper thighs twice daily for 12 weeksAverage cellulite score fell from 3.96 to 2.50 with active cream and from 3.88 to 2.83 with placebo.

The six week result was a partial improvement, not disappearance of cellulite. Three independent investigators graded thigh dimpling on a scale from 0 to 9. Average body weight did not change significantly, and one participant was excluded after losing 5 kg. Slight itching and temporary flushing were commonly reported, although no serious adverse event occurred.4

The 12 week trial used random allocation and blinded assessment, with each volunteer trying active cream and placebo. The active cream had a moderate effect size, while placebo had a small effect. Both thighs improved in the paired trial, so the plain cream deserves a seat at the meeting. Read the active result alongside the placebo result rather than treating the entire before and after change as caffeine's contribution.5

These were different formulas, durations and assessment methods, not a direct contest between 2% and 3.5% caffeine. The experimental formula was selected after skin penetration testing; its lanolin formulation delivered more caffeine through skin than a commercial caffeine product with the same concentration. Choose by the complete formula and its own results, not the largest percentage printed on the packaging.5

How should you judge reviews and photos?

Use reviews to ask precise questions rather than looking only for a dramatic photograph. Check whether the reviewer means less visible dimpling, a smaller measurement or a short lived smoothing effect. Many home and spa treatments only diminish dimples for several hours. An immediate after photo is therefore a poor reason to expect lasting change.1

For your own comparison, photograph the same area in the same position, without changing whether you pinch the skin. Cellulite can appear only when skin is pinched, and sitting or standing can change its visibility. Record visible dimpling separately from circumference. A smaller circumference is not the same as smoother dimples, even if both look convenient in an advert.23

Should you choose caffeine or retinol?

Caffeine is the option to consider if you are willing to apply a product every day for an appearance benefit that needs maintenance. It may dehydrate cells, making cellulite less obvious. Retinol is the slower option: 0.3% retinol may help thicken skin and reduce visible dimpling. Allow at least six months before deciding whether it helps. Neither is a permanent solution.2

How to try a cellulite cream

  1. 1Test the cream on a small area first. Wait 48 hours and use more only if there is no skin reaction.
  2. 2Follow the exact product's directions. The two caffeine trials used twice daily application; that schedule is not a universal direction for every cream.
  3. 3Take a baseline photo and record the area being treated. For measurements, use the same location each time, as the six week trial did.
  4. 4Compare results over weeks rather than after one application. The caffeine trials assessed six and 12 weeks; retinol needs at least six months.

Who should be careful?

Allergic skin reactions are possible. Avoid aminophylline products if you use an asthma inhaler: even topical aminophylline can cause breathing problems. Anxiety and a racing heart have also been reported. Stop using a cream that causes a reaction, and seek medical help for breathing problems or a racing heart.2

The six week caffeine trial excluded pregnant and breastfeeding participants, people with other skin disease on the treated areas, and people taking drugs or agents that could affect fat metabolism. Its reported tolerability applies to a selected group of healthy volunteers.4

When is a dermatologist more useful than another cream?

If you want a longer lasting change, discuss procedures with a dermatologist rather than expecting a cream to remove dimples. Laser treatment targeting connective bands has reduced cellulite's appearance for a year or longer; subcision releases those bands with a small needling device, with results lasting up to two years. These are cosmetic procedures, not permanent cures. Pain, swelling or easy bruising deserves medical assessment rather than treatment as ordinary cellulite, which does not hurt.13
