Why does chest acne happen?
The chest is a common acne site because it has many oil glands. Sebum, the skin's natural oil, and dead skin cells can block follicles. Inflammation then produces swollen or painful pimples. Tight clothing, high humidity, greasy lotions and picking can make breakouts worse. Chest acne is not automatically hormonal just because it occurs in a woman: several factors can contribute at once.12
Breakouts around your period can reflect changing hormone activity. Pregnancy can also trigger acne, which can continue after birth. During menopause, lower estrogen and higher androgen activity can increase oil production. Adult acne can persist into your 30s, 40s and 50s; it is not confined to adolescence.13
New breakouts can follow stopping estrogen containing birth control or taking medicines such as testosterone, corticosteroids, lithium or anticonvulsants. Discuss recent medication changes with your doctor. Acne alongside irregular periods or excessive hair growth is also worth mentioning, because hormonal disorders can contribute. Polycystic ovarian disease is one possible trigger, not a diagnosis you can make from chest spots alone.23
Is every chest breakout acne?
Chest acne can include blackheads, whiteheads, small raised bumps, pus filled pimples and larger cysts, sometimes with scars. Blackheads and whiteheads are blocked pores closer to the surface and usually are not sore. Inflamed pimples are swollen and tender; nodules are hard lumps beneath the skin, while cysts are softer, fluid filled lumps.45
An itchy, acne like eruption can instead be folliculitis, inflammation of hair follicles associated with yeast. This is often called "fungal acne." Have an uncertain or persistent eruption examined rather than assuming every chest bump needs the same treatment.1
How to start treating mild chest acne
Strong acne products can be too drying for aging skin. A cleanser containing salicylic acid is one option; prescription treatment is the next step when shop bought products are not helping.3
- 1Clean the chest gently in the morning and evening, and after exercise. Do not scrub.
- 2If using benzoyl peroxide, choose a product labelled for the chest and start with the lowest strength. For a wash, wet the skin, wash for 1 to 2 minutes, rinse and pat dry. For a gel left on the skin, clean and dry the area, then apply a thin layer. Wash your hands before and afterwards.
- 3Apply an oil free, water based, noncomedogenic moisturizer, meaning one formulated not to clog pores.
- 4Use an oil free, noncomedogenic sunscreen to protect exposed skin. Benzoyl peroxide increases sensitivity to sunlight.
- 5Leave pimples alone. Picking or squeezing can cause scarring, infection and more inflamed blemishes.
Which acne treatments do what?
Acne treatments come as washes and products left on the skin, so the word "treatment" on a bottle is doing rather broad administrative work. The ingredient and directions matter more than the category name.1
- Benzoyl peroxide targets bacteria that aggravate acne. Lower concentrations and wash formulations tend to be less irritating.
- Salicylic acid loosens dead skin cells to help prevent blocked follicles. It comes in acne cleansers and lotions.
- Retinoids, medicines related to vitamin A, help clear blackheads and whiteheads and prevent clogged pores. They treat the affected area rather than individual spots.
- Azelaic acid is another topical option for mild acne. Topical means applied directly to the skin.
- Moderate acne can require an oral antibiotic alongside topical treatment. Persistent or treatment resistant acne can require isotretinoin.
How long does treatment take?
Benzoyl peroxide usually starts working within four weeks; seek advice if acne worsens or has not improved after 12 weeks. Acne treatment more broadly can take two to three months to show improvement. Retinoids often need several months. Inflamed acne can also take months of treatment to settle. Continue a maintenance routine after improvement, because acne can return when treatment stops.1567
What if treatment irritates your chest?
Benzoyl peroxide is usually used once or twice daily. Dryness, peeling or itching can improve with less frequent use; persistent or bothersome symptoms need advice. More product does not work faster. Avoid broken or infected skin, and protect clothing and hair from bleaching. Combine it with other acne treatments only with pharmacist or doctor approval. Stop using it and seek emergency help for throat or tongue swelling or difficulty breathing.6
What about pregnancy and breastfeeding?
Benzoyl peroxide can usually be used during pregnancy, but check with your doctor and tell them if you become pregnant while using it. During breastfeeding, check with a pharmacist or doctor first. If approved, do not apply it to your breasts.6
Can hormonal treatment help?
For hormone driven acne, a dermatologist can consider certain birth control pills or spironolactone, which reduces excess oil. These are prescription decisions that depend on your medical history. Tell the dermatologist about all medical conditions, including heart disease when discussing the pill. Severe acne can need an oral antibiotic with topical medicine, or isotretinoin, which requires careful consideration of side effects and a monitoring program.37
When should you see a doctor?
Book an appointment for deep painful lumps, scarring, sudden severe adult acne or breakouts that persist despite treatment. Severe acne cannot be cleared with shop bought treatment alone. A dermatologist can treat a large painful cyst with an injection or drainage; do not drain it at home. Acne with fever, joint or bone pain, or extensive ulcerated skin needs urgent assessment. Most ordinary acne needs no tests, but unusual features can warrant skin swabs or hormonal testing.127
