Which types of acne can exfoliation help?

Acne develops when hair follicles become blocked with oil and dead skin cells. Blackheads and whiteheads are these plugs, also called comedones. Salicylic acid and glycolic acid are treatment options for comedonal acne, meaning acne made up of clogged pores. Salicylic acid is also available in acne washes, which can help oily skin prone to breakouts.123

Deep, painful acne needs treatment that addresses clogged pores, bacteria, inflammation and excess oil, not just surface exfoliation. Adapalene is another option for unclogging pores and preventing new blackheads. Choosing treatment for the type of blemish matters more than adding an exfoliation step to every routine.4

How should you exfoliate acne prone skin?

  1. 1Check your existing treatments first. Adding exfoliation to prescription retinoids, retinol or benzoyl peroxide can worsen dryness or cause breakouts.
  2. 2Choose a mild chemical exfoliator rather than a scrub or brush. Mechanical exfoliation can be too irritating for acne prone, dry or sensitive skin.
  3. 3Cleanse gently. Wash twice a day with a gentle cleanser, and avoid repeated washing or rubbing to remove excess oil.
  4. 4Apply without scrubbing. For a rinse off exfoliator, use gentle circular motions for about 30 seconds, then rinse with lukewarm water. Never exfoliate sunburned skin or open cuts.
  5. 5Apply moisturizer immediately afterwards. Choose a water based or noncomedogenic moisturizer to help with dryness.

There is no single exfoliation schedule for acne prone skin: frequency depends on skin type and method, and stronger exfoliation belongs less often. Morning and evening cleansing does not mean morning and evening exfoliation. If an acne medicine irritates your skin, using it every other or third day can help while your skin adjusts; waiting 10 to 15 minutes after washing before applying it can also help.35

Choose noncomedogenic skincare and makeup, meaning products intended not to clog pores. Noncomedogenic describes pore clogging, not a personality trait, however impressive the word looks on a bottle. Stop using a product that makes your acne worse, even if its packaging uses that term. Wear sunscreen daily, especially if your acne medicine makes your skin more sensitive to sunlight.123

How long does it take to help?

Acne treatment generally needs 6 to 8 weeks of consistent use before improvement appears. Blackheads and whiteheads can be slower: comedonal acne can start improving in 8 to 12 weeks and take many months to clear. These are treatment timelines, not a reason to keep exfoliating through irritation. Redness, burning or worsening dryness calls for backing off, not increasing frequency.145

When should you get medical help?

Avoid aggressive exfoliation if you have darker skin or develop dark marks after breakouts, because irritation can increase pigmentation. See a doctor for deep or painful spots, scarring, worsening acne, hot and swollen skin, or acne affecting your mental health. Get help if self treatment is not improving your acne rather than escalating to stronger exfoliation.25
