How do hormones affect chest acne?

Acne involves hair follicles and their oil glands. Androgens, a group of sex hormones, stimulate these glands. Oil, dead skin cells and inflammation contribute to blocked follicles, producing blackheads, whiteheads and inflamed spots. Hormones are part of this process, alongside genetics, bacteria and the skin's immune response.245

Hormonal changes during puberty, periods, pregnancy and menopause can contribute to acne. Adult hormonal acne can affect the chest, shoulders and back, not just the chin and jawline. Notice whether chest breakouts coincide with these changes, and tell your doctor about the pattern rather than assuming every chest spot needs hormonal treatment.14

Also mention new acne after stopping an estrogen containing contraceptive or starting testosterone treatment. Stress can aggravate acne through hormonal changes. Acne alongside irregular periods or excessive hair growth deserves a medical assessment because these can occur together with an underlying hormonal condition. Most acne does not need tests; unusual features can prompt hormonal testing.12

What else could be causing chest spots?

Sweat mixed with bacteria and skin oils can clog pores. Greasy skin or hair products, high humidity and pressure against the skin can also contribute. Corticosteroids, some seizure medicines and lithium can provoke acne, so include prescriptions and recent medication changes when discussing new breakouts with your doctor.135

Some chest bumps are not acne. Folliculitis is an infection of hair follicles that produces pimple like bumps, often itchy and tender. Heat rash comes from blocked sweat glands and causes tiny, itchy bumps. A rash that persists or worsens needs assessment rather than repeated changes of acne products.3

What can you do now?

  • Wash the affected skin twice daily with a gentle cleanser. Do not scrub or squeeze spots: scrubbing can worsen irritation, and squeezing can cause scars.
  • Ask a pharmacist about a topical treatment such as salicylic acid or benzoyl peroxide. For older, easily irritated skin, avoid reaching straight for the strongest product.
  • Choose products labelled noncomedogenic, meaning formulated not to clog pores, rather than choosing by how expensive the packaging looks.
  • Wear loose, breathable clothing when exercising or sweating. Blot sweat with a clean towel instead of rubbing it away.
  • Sweaty workout clothes need washing before reuse, even if they have been promoted to a dedicated outfit.

When does hormonal treatment make sense?

Women with stubborn chest or back acne can discuss hormonal therapy with a dermatologist. Options include certain birth control pills and spironolactone, a prescription medicine that acts on hormones. These are daily medicines, not automatic treatments for every chest breakout. A clinician checks your health history, medicines and supplements before prescribing.7

Birth control pills carry risks including blood clots and high blood pressure. Spironolactone is not used for acne in men because of side effects. If you can become pregnant, contraception is essential while taking spironolactone because exposure during pregnancy can cause serious birth defects. Avoid potassium supplements while taking it.7

Improvement with the pill usually takes 2 to 3 months. Spironolactone can reduce breakouts and oiliness within a few weeks. Keep follow up appointments and contact your dermatologist immediately for signs of a serious side effect, such as cramping in an arm or leg.7

When should you see a doctor?

Seek help if self treatment is not working, spots are deep or painful, scars are developing, or acne affects your mental health. Hot, swollen or red skin also needs medical assessment; redness can be harder to see on brown and black skin.6
