Antifungal activity is not the same as a treatment

Witch hazel extracts have shown activity against fungi, bacteria and some viruses. However, strong clinical data supporting witch hazel's effectiveness remain limited, which leaves 'antifungal' looking better as a headline than a supporting document. There is no established witch hazel strength, application schedule or clearing time for a fungal skin infection. The practical distinction is between an extract with antimicrobial activity and a medicine used to treat an infection.1

Witch hazel also has antibacterial activity, including effects against certain staphylococci. Antibacterial and antifungal are not interchangeable terms: they refer to different targets, despite sharing enough letters to look like a matching set. Witch hazel has been combined with germicidal dairy teat dips to improve antibacterial activity, which is a rather indirect route to the bathroom shelf.3

What is bottled witch hazel for?

Bottled witch hazel and research extracts are not interchangeable, which makes 'witch hazel' a rather imprecise shopping list. As one example, Leader Witch Hazel liquid is an astringent for relief of minor irritation from insect bites, cuts and scrapes. An astringent and an antifungal medicine have different jobs.24

Leader tells you to apply its liquid as often as necessary for those minor irritations, not as a fungal treatment schedule. Use it externally, avoid swallowing and keep it away from your eyes. Stop and ask a doctor if the condition worsens or symptoms last longer than seven days. Ask a health professional before use during pregnancy or breastfeeding.4

What should you do instead?

  1. 1Speak to a pharmacist if you think you have a fungal infection. The location matters when choosing a cream, gel, spray or medicine taken by mouth.
  2. 2Check suitability before starting. Discuss pregnancy, breastfeeding, allergies, existing conditions and other medicines with a pharmacist or doctor.
  3. 3Use the chosen medicine for the full instructed course, even if the rash disappears. Stopping early can leave ringworm uncleared and harder to treat.
  4. 4For ringworm, wash your hands after treatment, wash towels and bedsheets regularly, and do not share towels, combs or bedding.

How long does antifungal treatment take?

InfectionTypical treatment and timing
Ringworm on the skinAntifungal cream or ointment, commonly twice daily for two to four weeks.
Mild athlete's footAn antifungal cream or spray can usually clear it in two weeks.
Jock itchAn antifungal cream, spray or powder, typically twice daily for 10 to 14 days.
Scalp, beard or nail ringwormPrescription treatment is needed. Nail infections take time to clear because nails grow slowly.
6

Follow your medicine's own directions rather than treating these timings as instructions for every product. As skin ringworm clears, scaling often disappears before redness. If ringworm is present in several places, all affected areas need treatment at the same time.6

When should you get medical help?

See a doctor for suspected scalp ringworm, a weakened immune system, or ringworm that has not improved with pharmacist recommended treatment. Severe athlete's foot, or athlete's foot that has not cleared in two weeks, also needs assessment. A dermatologist can examine the area and, when needed, test a skin, hair or nail sample for fungi.56

Antifungal medicines can cause itching, burning, redness or a rash. Stop the medicine and seek medical help for a severe reaction, including peeling or blistering skin or swelling of the face, neck or tongue. Difficulty breathing requires emergency help.2
