What is in triple combination cream?
A commonly studied formula contains hydroquinone 4%, tretinoin 0.05% and fluocinolone acetonide 0.01%; other combinations use different corticosteroids, so the name alone does not identify the formula. The three ingredients have different jobs rather than simply doing the same thing three times.23
| Ingredient | Role in treatment |
|---|---|
| Hydroquinone | Reduces production of melanin, the pigment that gives skin its color, by blocking an enzyme involved in making it. |
| Tretinoin | A retinoid that increases skin cell turnover and helps hydroquinone penetrate the skin. |
| Fluocinolone acetonide | A corticosteroid that reduces inflammation and irritation caused by the other ingredients. |
Who is this treatment for?
Start with a diagnosis rather than treating every dark patch as melasma. Melasma often affects the cheeks, forehead and upper lip, but pigmentation after inflammation, age spots and other conditions can resemble it. Melasma itself is not cancerous, although some skin cancers can mimic its appearance. A dermatologist can examine the patches and, occasionally, take a small skin sample to clarify the diagnosis.45
The depth of the pigment affects the response. Epidermal melasma, with pigment in the outer skin layer, usually responds better to topical treatment. Dermal melasma, with pigment deeper in the skin, responds poorly. Mixed melasma contains both and usually improves only partly. Skin tone, pigment depth and triggers help determine whether a dermatologist chooses triple combination cream or another treatment.135
How do you apply it?
Follow the directions for your prescribed cream. One studied regimen used Januluma cream, containing the strengths above, every night for eight weeks. That is a specific treatment course, not a reason to use any triple combination cream indefinitely.2
- 1Wash with a mild cleanser.
- 2Apply the prescribed cream to the melasma patches. In the Januluma regimen, application was 30 minutes before bedtime.
- 3Use moisturizer as part of the routine.
- 4During the day, apply broad spectrum sunscreen with SPF 30 or higher and continue sun protection throughout the day.
When medication, sunscreen and camouflage makeup are used together, the order is medication, sunscreen, then makeup. Camouflage makeup can hide patches while treatment works.1
Which sunscreen should you use?
Ultraviolet and visible light promote pigment production. Choose a broad spectrum sunscreen containing iron oxides; SPF 50 or higher is a useful choice for melasma. Tinted sunscreen is an option if a white cast makes sunscreen difficult to wear, which spares the sunscreen a second career as face paint. Seek shade as well. A broad brimmed hat helps with sun protection, even if its main ambition was to be an accessory.15
How long does it take to work?
Early improvement can appear within several weeks. In a small study of 22 women using Januluma nightly, melasma severity improved by four weeks and improved further by eight weeks. Those results describe that particular cream and group, not a fixed deadline for every prescription.2
A complete melasma treatment plan usually takes three to twelve months to show results, and longstanding melasma can take longer. That broader timetable is not an instruction to keep using the same combination cream for twelve months. The aim is to reduce excess pigment and restore your natural skin color, rather than lighten unaffected skin.1
What side effects should you watch for?
Itching, scaling and redness occurred in almost all participants in the Januluma study, especially during the first month, and were generally mild. The corticosteroid reduces irritation but does not eliminate it. If symptoms are troublesome, contact your prescriber rather than increasing application or adding more treatments.2
Products that irritate skin can worsen melasma. If your routine is causing irritation, have the products reviewed rather than treating discomfort as proof of progress. A dermatologist can choose a gentler option, such as azelaic acid, when appropriate.14
Prolonged hydroquinone use can cause unwanted loss of pigment or exogenous ochronosis, an abnormal darkening of the skin. New darkening or areas losing their color need medical review, not more cream. Hydroquinone can also oxidize: a formulation that changes from white to brown should be discarded.3
What about pregnancy and other prescriptions?
Pregnancy, hormonal contraception and some medications can trigger melasma. Tell your dermatologist about pregnancy and the medicines you take before choosing treatment. Pregnancy related melasma can fade after delivery. Discuss a suspected medication trigger with its prescriber rather than stopping a prescription yourself.4
What happens after the cream course?
Melasma frequently returns, including after successful treatment, so continue sun protection throughout the year. A long term management plan does not necessarily mean long term use of the same medication. Have persistent patches reassessed rather than automatically extending the cream course or booking a peel. Peels and lasers can worsen melasma or cause pigmentation after inflammation, and require expert selection.5
