Where does hormonal acne appear on the face?
The chin, jawline and area around the mouth are typical locations for adult hormonal breakouts. Cheeks are another common site, and hormonal acne can occur elsewhere on the face. Period related breakouts often cluster on the lower face, but their location is not always identical from one person to another. Hormonal acne can include blackheads, whiteheads and pimples as well as deeper, painful cysts. Some period related bumps never develop a visible head.234
Can hormonal acne occur on the body?
Yes. The chest, shoulders and back are recognised locations for hormonal acne, and the neck can also be affected. Acne commonly develops in areas with more oil glands. Having body acne does not, by itself, identify the cause: tight clothing, greasy products and medication effects can also contribute to breakouts.45
Does jawline acne mean your hormones are out of balance?
Not necessarily. Androgens, a group of hormones that includes testosterone, stimulate the skin's oil glands. Increased oil production contributes to clogged pores, alongside dead skin cells and bacteria. The glands can also be unusually sensitive to normal hormone levels, so hormonal involvement does not automatically mean that hormone levels are high.136
Location is one clue, not a diagnosis. A clinician also considers the timing of breakouts, medication use, health changes and response to treatment. Hormone testing is not needed for every person with acne, particularly when ordinary treatment works. New, severe or persistent adult acne deserves assessment rather than a diagnosis based only on where the spots appear.1356
What other clues should you look for?
Repeated flares before a period can suggest a hormonal contribution, although breakouts can happen at other points in the menstrual cycle. Period acne can last a few days to a few weeks. Pregnancy, menopause, stopping birth control and testosterone treatment can also be associated with acne. Hormonal acne affects both men and women.23
Tell your healthcare provider if acne comes with irregular periods or increased body hair, because these symptoms can prompt checks for an underlying hormonal condition. Also mention medicines you have started or stopped. Your skincare product names belong in the appointment notes, even if 'radiance' has to appear several times.234
What should you do about these breakouts?
Cleanse daily with a gentle cleanser, avoid scrubbing and do not pick or squeeze spots. Choose products labelled noncomedogenic, meaning designed not to clog pores, rather than letting an ambitious product name make the decision. Strong acne products can be too drying for adult skin. For sensitive skin, lower concentrations of benzoyl peroxide are less irritating.2345
See a dermatologist if acne persists, worsens or develops deep, painful lumps; cystic and nodular acne can scar. Women with stubborn lower face, jawline or neck acne can discuss prescription hormonal treatment. Birth control pills typically take 2 to 3 months to improve acne, while spironolactone can reduce breakouts and oiliness within a few weeks.357
If you are pregnant or planning pregnancy, ask your healthcare provider to choose an appropriate acne treatment. Avoid topical retinoids and isotretinoin during pregnancy. Spironolactone requires birth control if you can become pregnant because exposure during pregnancy can cause serious birth defects.37
