Give one active a fair trial

Choose one over the counter product with an active ingredient suited to your kind of acne. Follow its directions; the bathroom shelf does not need to run an audition. Use it consistently for 6 to 8 weeks before deciding it has failed. Adding another acne treatment during that trial can make it harder to tell what is helping and can add irritation.1

When a treatment works for you, fewer breakouts can appear in 4 to 8 weeks, while clearing can take 16 weeks. One week is too soon to judge the result. Retinoids often take several months before you notice improvement, so the expected timeline depends on the treatment.13

Choose an active ingredient that fits the kind of acne you are treating. These are alternatives, not a set to start together; the list is a menu, and your skin does not need the whole kitchen at once.

  • Adapalene, a retinoid, helps clear blackheads, whiteheads and pimples.
  • Benzoyl peroxide is especially effective for mild pimples.
  • Salicylic acid works best on whiteheads and blackheads.
  • Azelaic acid fights acne and can also fade the dark spots left after a spot clears.

Apply a retinoid across the whole acne prone area rather than only to visible spots. Benzoyl peroxide and topical retinoids can irritate skin, so introduce them slowly, starting at low strength and low frequency. Hydrating products can help. If irritation makes use difficult, short contact at first, such as washing the product off after an hour, can help build tolerance. Scrubbing can irritate skin, and picking or squeezing spots can cause scars, so keep cleansing gentle.1345

Why can acne return?

Hormone changes, including around periods, can trigger acne. Stress, high humidity, oily personal care products, certain medicines and skin or hair products that make skin greasy can also contribute to breakouts. High humidity and greasy products can worsen acne, so the cleanser is not the only department in this meeting. If acne changes after you start a medicine, tell the clinician who prescribed it, since some medicines can contribute to acne.35

Can acne products stop working over time?

A flare alone does not tell you why acne has returned. Acne can persist or flare because it is a chronic condition and because triggers change. Antibiotic resistance is a specific concern in acne care: benzoyl peroxide can be used alongside topical or oral antibiotics to reduce resistant C. acnes. If acne worsens while you are using a prescription antibiotic, ask the prescriber to review the plan rather than changing prescription treatment yourself.4

Why is my acne getting worse despite treatment?

The treatment may not match the current acne. Over the counter products are unlikely to treat large, painful pimples deep in the skin or acne that leaves scars effectively. Deep nodules and cysts can scar, so seek medical help rather than waiting for a mild acne product to clear them. For women with persistent acne along the lower face or jawline, a dermatologist can discuss whether hormonal therapy is an option.136

How do I know when to change my acne routine?

If mild acne is not improving after 6 to 8 weeks of using one suitable product as directed, arrange an appointment with a GP or dermatologist instead of cycling through more products. Get help sooner if you have many spots, deep or painful spots, scarring, or acne that is affecting your mental health. If you are pregnant or trying to get pregnant, consult a dermatologist before treating acne on your own. Topical retinoids should not be prescribed during pregnancy or breastfeeding.145
