What makes skin lose its support?
Sagging is not just a surface change. Aging weakens the skin's connective tissue, reducing strength and elasticity. Loss of fat beneath the cheeks, temples, chin and eyes also changes facial fullness. Bone loss can contribute to slack skin around the mouth after age 50. Once skin loses elasticity, gravity contributes to drooping eyelids and looseness around the cheeks and jaw.2
Ultraviolet light from the sun damages elastin, leaving skin less able to spring back after stretching. That damage builds over time and can become visible much later, so a recent change in appearance does not necessarily mean the damage happened recently. Compare areas regularly exposed to sunlight with covered areas: sun exposure is the biggest environmental contributor to aging skin.12
Genes largely control when natural aging changes appear. Smoking speeds up skin aging, while hormonal changes around menopause can make skin thinner and drier. These changes can happen together, but dryness, wrinkles and loose skin are not interchangeable problems. Chronic illnesses and reactions to medicines can also cause skin changes, which is another reason not to diagnose an abrupt change from appearance alone.134
What can you do now?
- Protect exposed skin every day with broad spectrum, water resistant SPF 30 or higher sunscreen. Apply it 15 to 30 minutes before going outside and reapply at least every two hours, more often after swimming or heavy sweating.
- Use shade, protective clothing and a wide brimmed hat, and avoid tanning beds. Sunscreen is one part of protection, not a substitute for covering up.
- If you smoke, stop. Smoking accelerates skin aging, and people who stop often notice healthier looking skin.
- Wash gently rather than scrubbing. Use warm instead of hot water and avoid strongly drying or heavily perfumed soaps.
- Stop ordinary skincare products that sting or burn. If a prescribed aging treatment causes these symptoms, tell the prescribing dermatologist.
Apply moisturizer every day, particularly if your skin feels dry or rough. Sun protection can slow further damage and give skin a chance to repair some existing damage, but it cannot completely undo sun damage.24
Can a cream or treatment tighten sagging skin?
Do not choose a cream solely because its packaging promises younger skin. Many antiaging products are cosmetics and do not require human testing or FDA approval before sale, leaving the product name to do a suspicious amount of work. A dermatologist can help separate care for dryness and wrinkles from treatment aimed at tightening loose skin.34
Professional microneedling is one option used for sagging skin. It creates controlled tiny injuries that stimulate new collagen and elastin, improving firmness and texture. Multiple sessions can be needed, with at least six weeks between treatments. Skin rejuvenation can appear from two weeks to six to eight months after treatment; that is not a fixed timetable for correcting sagging.6
Microneedling can cause redness, dryness, scaling and swelling for several days or longer. Infection, cold sore reactivation and other reactions are possible. Discuss suitability and recovery with the treating clinician before booking, rather than treating needling as another everyday skincare step.6
When should you get the change checked?
For an unusually fast change, tell your doctor when it started, which areas changed and whether other symptoms appeared. Some conditions affecting elastic fibers, including cutis laxa, can cause loose skin that resembles premature aging. Cutis laxa has inherited and acquired forms; appearance alone is not a diagnosis. Also seek assessment for unexplained bruising or very dry, itchy skin, especially if itching disrupts sleep.237
