Choose adapalene when treating acne is the main job. Consider retinol when mild uneven pigmentation, texture or fine lines are the main concern. Adapalene comes in both prescription and nonprescription forms.13

A percentage on a retinol bottle is not enough to choose your treatment, which is inconvenient for packaging that would rather be a scoreboard. Retinol percentages do not have to appear on every label, and beginners should look for the least intense formula available rather than immediately choosing the strongest option.1

How should you start?

  1. 1Check the directions first. For retinol, test a small patch and wait a couple of days before starting if the patch is not very red or itchy.
  2. 2At night, wash gently and pat dry. Do not scrub or use abrasive cleansers.
  3. 3Apply a thin layer. Spread adapalene over the entire acne affected area, not just individual pimples. For facial retinol, general guidance is a pea sized amount, avoiding the eyes, nose and mouth.
  4. 4Follow the adapalene label or prescription schedule, usually once daily at bedtime. Start retinol every other night and increase gradually as tolerated.
  5. 5Use moisturizer. If prescribed acne treatment irritates your skin, ask your dermatologist about applying moisturizer first or adjusting the amount or frequency.
  6. 6During the day, use sunscreen with broad spectrum protection, SPF 30 or higher and water resistance, alongside shade and protective clothing.

What should you expect?

Acne treatment generally takes at least 6 to 8 weeks before fewer breakouts appear. With adapalene, acne can initially worsen as previously unseen pimples reach the surface. Early breakouts do not automatically mean treatment has failed. Applying extra adapalene will not speed up improvement and can irritate your skin.35

Who should be careful?

Both options can cause dryness, redness and peeling. Sensitive, very dry or allergy prone skin needs particular caution. Irritation can trigger dark marks in darker skin. Do not use retinoids during pregnancy; discuss planned pregnancy or breastfeeding with your clinician. Do not apply adapalene to sunburned, broken or eczema affected skin until it heals.136

Tell your doctor about your medicines and skincare before starting adapalene. Harsh or drying products can add irritation, and hot waxing should be avoided on treated areas. Contact your doctor for burning, stinging or irritation, especially if severe or persistent. Get urgent medical help for hives, breathing difficulty or swelling of the face, lips, tongue or throat.3

Is a retinoid stronger than retinol?

Not necessarily: retinol is itself a retinoid, the umbrella term for vitamin A related ingredients and medicines. Retinol and adapalene belong to that family, so comparing "retinoid" with "retinol" asks the family name to do a product's job, which is how a shopping decision becomes a genealogy assignment. The specific medicine, formula and treatment goal matter.67

Is tretinoin stronger than adapalene?

There is no single strength ranking to apply to every formula, however neatly a podium would fit on the bathroom shelf. Both treat acne, while tretinoin is prescription only and also improves fine lines and wrinkles. If acne is leaving substantial scars or persists despite treatment, see a dermatologist rather than choosing a new medicine by its reputation for strength.14
