Can Centella help with rosacea?

Centella asiatica, also called gotu kola, is a plant studied for wound healing and effects on inflammation during healing. That is a reason for interest in skincare, not a reason to expect it to control rosacea redness, flushing or bumps. Its dermatology research focuses on small wounds, scars and burns rather than rosacea. There is no rosacea specific strength or improvement timetable to use here. A Centella product is still a skincare product, even if the packaging gives the plant top billing.23

Which Centella formula should you choose?

Use general rosacea skincare guidance to choose the finished product. Look for fragrance free products made for sensitive skin, preferably labelled noncomedogenic, meaning they do not clog pores. Avoid formulas containing alcohol, camphor, fragrance, glycolic acid, lactic acid, menthol, sodium laurel sulfate or urea. Witch hazel, peppermint and eucalyptus oil are also reported rosacea irritants. An 'unscented' label does not necessarily mean fragrance free, and botanical products can contain fragrance related ingredients. If a particular ingredient has already caused a reaction, avoid it rather than testing your luck with another formula containing it.145

How do you test a Centella product?

  1. 1Choose a patch about the size of a quarter on the underside of your arm or at the bend of your elbow.
  2. 2Apply the normal amount and thickness twice daily for seven to 10 days.
  3. 3Leave the product on for its usual contact time. For a cleanser, follow its instructions or rinse after five minutes.
  4. 4If the area stays free of redness, itching and swelling, you can begin using the product as directed.
5

This is a home tolerance test, not medical patch testing. It helps predict a reaction before you use a product more widely. If reactions keep happening and you cannot identify the cause, a dermatologist can arrange medical patch testing to help identify the responsible ingredient.5

Where does Centella fit in your routine?

Follow the finished product's directions rather than treating Centella as an ingredient with one universal application schedule. If you choose a Centella moisturizer, use it after gentle cleansing and moisturize every day, including when your skin feels oily. A moisturizer containing Centella can fill the moisturizer slot; the ingredient does not need a separate appointment on the bathroom shelf. Keep the number of products low to reduce opportunities for irritation. Avoid rubbing, scrubbing and exfoliating. During the day, use a broad spectrum, water resistant sunscreen with SPF 30 or higher; zinc oxide, titanium dioxide or both are suitable ingredients to look for.14

When should you stop or get medical help?

Stop using the product if it causes a reaction, including burning, stinging, itching, swelling or increased redness. Gently wash it off. A cool compress or petroleum jelly can help relieve a skin reaction. See a dermatologist if the reaction is severe or these measures do not relieve it. Burning and stinging can also be rosacea symptoms, so persistent discomfort needs attention rather than repeated product trials.56

If skincare changes do not control symptoms, seek a rosacea treatment plan rather than replacing prescribed treatment with Centella. Some topical rosacea medicines take at least eight weeks to show an effect; that timing does not apply to Centella. Get urgent medical advice for eye pain, blurred vision, light sensitivity, a red eye or a gritty feeling in the eye.167
