How does adapalene work?

Acne starts when oil and skin cells form a plug inside a pore. A plugged pore can become a blackhead or whitehead; inflammation can produce red bumps and pimples. Adapalene increases skin cell turnover, helping the skin shed cells and unblock pores so oil can escape. That reduces the formation of blackheads and spots. Apply adapalene across the affected area rather than only to visible pimples: treating the whole area helps address existing acne and prevent the next breakout.134

Who is adapalene useful for?

Adapalene is used for acne in adults and children aged 12 and older. Mild acne with blackheads, whiteheads and small pimples is a reasonable starting point for home treatment. Adapalene gel is among the treatments used for mild acne, while more severe acne can require additional medicines. Deep, painful lumps, cysts or acne that leaves scars need medical assessment rather than relying on a shop bought product alone.125

Adapalene comes alone or combined with benzoyl peroxide. Differin cream and gel contain 0.1% adapalene; Epiduo combinations include 0.1% or 0.3% adapalene with 2.5% benzoyl peroxide. Follow the directions for your actual product, especially a prescribed combination. Choosing a treatment by its active ingredient is more useful than choosing by its product name, however much confidence the box has in its typography.136

What about acne marks, scars and wrinkles?

As a group, topical retinoids can improve postinflammatory hyperpigmentation, the dark marks left after inflammation, and may reduce existing and future acne scarring. Those are broader retinoid benefits, not a promise that adapalene will erase a particular scar or mark. Adapalene's clear role here is acne treatment; tretinoin has the more specific role in treating sun damage and fine wrinkles. Persistent dark marks or developing scars are reasons to get a treatment plan from a dermatologist.17

How often should you use adapalene?

The usual schedule is once daily at bedtime. To ease into treatment, you can start two to three nights per week and gradually build towards nightly use as tolerated, following your product or prescriber's instructions. General acne guidance also allows alternate day use or short contact application at the start to reduce irritation. Do not apply adapalene more than once daily or use extra to accelerate results.236

How do you apply and layer adapalene?

  1. 1Remove makeup and gently wash with a mild cleanser. Avoid scrubbing, then pat the skin dry.
  2. 2With clean fingertips, spread a thin film of adapalene cream or gel over the whole affected area. Keep it away from eyelids, lips, eyes, nose and mouth.
  3. 3Wash your hands after applying. If cream or gel reaches an area you should avoid, wash it off with warm water.
  4. 4Let adapalene absorb before applying moisturizer or cosmetics.
  5. 5During the day, protect treated skin with sunscreen and clothing. SPF 50 is advised in the NHS adapalene instructions, including on cloudy days.

What can you use alongside adapalene?

Start with one acne treatment rather than adding another during the first trial, because a crowded bathroom shelf is not the same thing as a plan. Use it as directed for six to eight weeks before adding another product. A gentle cleanser and a noncomedogenic moisturizer, designed not to clog pores in most people, support a simple routine. Avoid harsh exfoliation and repeated washing; wash no more than twice daily.146

Avoid other retinoid products, salicylic acid or glycolic acid peeling products, abrasive cleansers and strongly drying products while using adapalene. These can add to irritation. Benzoyl peroxide, clindamycin or erythromycin can be used alongside it when appropriate; separate products can be applied at different times of day. A prescribed combination such as Epiduo already contains benzoyl peroxide. Check new medicines and your existing skincare with a pharmacist before combining treatments.3

Can adapalene dry out your skin?

Yes. Dryness, redness, scaling, burning and stinging are possible, especially during the first two to four weeks. These effects often ease as the skin adjusts. Moisturizer and less frequent application can help. If skin becomes very irritated, pause treatment; stop and contact your doctor if irritation is severe or does not improve. Do not apply adapalene to broken, sunburned or eczema affected skin. Avoid hot waxing on treated areas and avoid tanning beds.23

How long does adapalene take to work?

Allow eight to twelve weeks or longer for the full benefit. Acne can look worse in the first few weeks as previously unseen pimples reach the surface. Early worsening alone does not mean you should stop, but irritation still needs attention. General acne treatment timelines put fewer breakouts at four to eight weeks and clearing around sixteen weeks when treatment works. A twelve week review is useful for deciding whether to continue or change the plan. Adapalene controls acne rather than permanently curing it.126

When should you avoid adapalene or get help?

Do not start adapalene during pregnancy or while trying to become pregnant. Stop treatment and contact your doctor if you become pregnant or suspect pregnancy. If breastfeeding, ask your prescriber before using it; do not apply it to the nipple or areola. Tell your doctor about eczema, other skin conditions, allergies and medicines before starting.123

Stop using adapalene and get urgent medical help for hives, difficulty breathing or swelling of the eyes, face, lips, tongue or throat. Arrange an acne review sooner for worsening acne, cysts, scarring, persistent dark marks or emotional distress. If consistent treatment has not helped after a few months, see a clinician rather than repeatedly adding more products.24
